By the time you meet Lorath Nahr in Diablo IV, he's a lonely, aged hermit living in a shack and one of the few Horadrim members left in the entire world of Sanctuary. He acts as a Deckard Cain-type character — scholarly and knowledgeable — helping lead the hero toward victory against Lilith, the Mother of the Prime Evils. He's become a bitter old man and a reclusive alcoholic, yet remains one of the most compelling characters in the game.

Neyrelle is a sharp-minded scholar with a deep interest in the Horadric Order. She read everything she could find about the original Horadric Order and the lore surrounding Sanctuary's greatest threats — the kind of child who read books that bored others. Sharp-tongued before she was brave, her tenacity ultimately defined her. She is most remembered for the decision to bind Mephisto, the Lord of Hatred, within a soulstone and carry it — not to wield it or destroy it, but to contain it — believing her will and intellect could resist his corruption.

Lilith is the Mother of Sanctuary and daughter of Mephisto, the Lord of Hatred. She awakens burning urges in the hearts of those around her — be it demon, human, or beast. She fell in love with the Archangel Inarius, and together they created the human world of Sanctuary as a place for their offspring to live in peace, free from the interference of angels or demons. Their hybrid children became a new race known as the Nephalem.

For the endgame Echo fight, you need 1x Crux of the False Prophet as a Hoard Key to unlock his loot hoard, plus a cosmetic reward.

Mephisto is the Lord of Hatred, one of the Three Prime Evils of the Diablo franchise. In Diablo IV's "Lord of Hatred" expansion, he disguises himself as a prophet of Akarat, manipulating people from the shadows, until he's revealed and stabbed with Lilith's Blade, after which he descends into the Pools of Creation and rises in his true demonic form. The fight has multiple phases — he emerges from a corrupted pool and attacks with a Meteor rain of fireballs, a Torrent attack where he crushes civilians and unleashes their gore, and an icy Beam that leaves a trail of Frostbite. As the fight progresses, his lower half becomes submerged in a pool of blood, and at about one-third health he pulls his disciples into the pool and begins casting Blood Fissures across the ground.
