Mephisto

Mephisto

Physical, Shadow, Lightning, and Cold damage. 1 game Boss in 1 game
Mephisto is the Lord of Hatred, one of the Three Prime Evils of the Diablo franchise. In Diablo IV's "Lord of Hatred" expansion, he disguises himself as a prophet of Akarat, manipulating people from the shadows, until he's revealed and stabbed with Lilith's Blade, after which he descends into the Pools of Creation and rises in his true demonic form. The fight has multiple phases — he emerges from a corrupted pool and attacks with a Meteor rain of fireballs, a Torrent attack where he crushes civilians and unleashes their gore, and an icy Beam that leaves a trail of Frostbite. As the fight progresses, his lower half becomes submerged in a pool of blood, and at about one-third health he pulls his disciples into the pool and begins casting Blood Fissures across the ground.

This is a very recent boss — Mephisto was added with the Lord of Hatred expansion (around early-mid 2026), so a lot of community knowledge is still evolving.
Players generally consider it one of the hardest fights in the game, with unskippable cutscenes and a reaction-heavy, "split-second one-shot mechanic" being common complaints. Blizzard Forums
The community recommends an S-Tier Boss Killer build, since most of his abilities can one-shot an average player build — gear like "Endurant Faith" that converts hits into damage-over-time is recommended to survive. Icy Veins
His attacks are heavily telegraphed, so the fight is largely about pattern recognition and clean dodging rather than raw DPS races.
